Account Application Form

You must have an account application form filled and signed for Directron.com to accept Purchase Orders and company checks as payment methods.

You have a few easy ways to obtain a copy of Directron account application form:

• View and print the online form in PDF format (Adobe Acrobat Reader required)
• View and print the online form in Word format

Fax the completed form to 281-754-4142. In addition to the completed account application form, the following are necessary for a speedy acceptance of your account application:

• For resellers, your resale certificate from your State. Click here for Texas Sales and Use Tax Resale Certificate, print and fill out only the first page. Adobe Acrobat Reader is required to fill out and print the form. Fax the completed form to 281-754-4142.
• For charity organizations, schools, government agencies, and non-profit organizations in Texas, please fill out the 2nd page of the Texas Sales and Use Tax Resale Certificate. Adobe Acrobat Reader is required to fill out and print the form.
• A cover letter in company/business/government letter head.
• Any additional credit information.
